This On-line Form is for your convenience and is to be used for non-urgent requests, ONLY. If you prefer to speak with someone immediately, please call your provider's office during normal business hours (Monday-Friday from 9:00am to 5:00pm except Holidays).
If you are submitting this request for non-urgent appointments (for example: new patients, annual physicals, non-urgent conditions, or well visits) during non-business hours, please be advised that the on-line appointment request forms are NOT monitored or reviewed during non-business hours and will NOT be seen until normal business hours on the following business day (Monday-Friday from 9:00am to 5:00pm except Holidays).
Once your request is received we will contact you to let you know if your requested appointment date/time is available.
No Show Fee Policy
If you do not give at least 48 hours’ notice of cancellation of your scheduled appointment, a no-show fee may be billed to your account.
